Percent of clients supported through APH, including through community-based preventative health screenings, who follow through with referrals
Measure Description
Preventative health screening services are provided through the Quality Of Life mobile outreach public health nurses and Neighborhood Services public health nurses located at neighborhood centers. These screenings include fasting glucose tests, blood pressure monitoring, and other services. Clients identified as having concerning health screening results are referred by the nurses to additional community services and/or healthcare providers. Staff follow up with each client to ensure the client connects with the referred service.
