Community Health Worker III

Purpose:
 

Under general supervision, this position provides culturally congruent health education and navigation services. Community Health Workers (CHW) will work with program staff and community providers to improve health outcomes.

Duties, Functions and Responsibilities:
  Essential duties and functions, pursuant to the Americans with Disabilities Act, may include the following. Other related duties may be assigned.
  1. Provides culturally congruent health education to clients and groups.
  2. Assists clients in accessing community resources and other basic need support.
  3. Identifies client needs and connects client to appropriate resources.
  4. Identifies, documents, and addresses cultural and socioeconomic barriers between clients and providers.
  5. Identifies target population gatekeepers, outreach sites, and conduct community outreach as requested.
  6. Provides continuous coaching and support for clients to sustain active participation in their personal health management. Responsibility may include performing home visits and assisting clients in appointments with primary care providers or other service providers.
  7. Maintains electronic health records and client information in accordance with HIPPA privacy standards.
  8. Provides classes and supervises clinical experiences that prepare students for CHW certification.
  9. Facilitates case conferencing with CHW team to discuss client cases and provide support.
  10. Expands knowledge and understanding of community resources, services, and programs.

  • Knowledge of community and family environmental conditions that affect a client’s social adjustment.
  • Knowledge of counseling skills in a culturally and linguistically acceptable manner for the target population to be served.
  • Knowledge of principles and practices of case management.
  • Knowledge of public health and social service resources.
  • Knowledge of personal computers and word processing software.
  • Knowledge of reporting procedures.
  • Skill in communicating both orally and in writing.
  • Skill in establishing and maintaining effective working relationships with supervisors, co-workers, officials, clients, providers, regulatory agencies, and the public.

  • Ability to maintain strict confidentiality as required by HIPAA and Texas privacy laws.
  • Ability to interpret rules and regulations regarding eligibility for other programs.
  • Ability to facilitate a variety of groups.
Minimum Qualifications:
 

Graduation from an accredited high school or equivalent, plus four (4) years of related experience providing services in a community setting, including three (3) years of experience working as a Community Health Worker.

Licenses and Certifications Required:
 
  • Must possess a State of Texas Community Health Worker certification.
  • State of Texas Community Health Worker Instructor certification required within six (6) months of hire.
